Ramipril (Hoe 498) is an angiotensin-converting enzyme (ACE) inhibitor. Ramipril is a prodrug which is rapidly hydrolyzed to the active metabolite ramiprilat by hepatic cleavage of the ester group. ACE, a peptidyl dipeptidase, circulates in the plasma and is located on the surface of endothelial cells. ACE is catalyzes the conversion of angiotensin I to the vasoconstrictor substance, angiotensin II. Thereby, ACE inhibitors reduce peripheral vascular resistance and blood pressure.
