Neostigmine methyl sulfate, a synthetic physostigmine analog, is a competitive cholinesterase inhibitor. Unlike the plant alkaloid physostigmine [
P0406], neostigmine methyl sulfate has a quaternary nitrogen, hence it more polar and does not cross the blood-brain barrier. Neostigmine methyl sulfate induces an increase in acetylcholine (ACh) in the synaptic cleft through reversible inhibition of its hydrolysis by acetylcholinesterase (AChE), and activates peripheral cholinergic nervous system.
